Project Objectives:
This project integrates artificial intelligence (AI) into medication reconciliation to identify high-risk patients with diabetes, ensuring more accurate and targeted interventions.
Target Population:
Patients with diabetes receiving care at UAB Medical Center or Cooper Green Mercy Health Services—particularly those managing complex medication regimens or affected by social determinants of health during post-discharge care transitions.
Relevance to Community:
Medication discrepancies contribute to poor diabetes outcomes, particularly among patients with health literacy challenges or financial constraints. This project addresses these gaps by optimizing resource allocation through AI-guided risk stratification.
Intervention/Approach
Strategies Used:
Develop machine learning models to identify patients at high risk for medication discrepancies.
Incorporate social determinants of health data into risk stratification.
Pilot the AI system within routine hospital workflows.
Cultural Relevance:
The project acknowledges disparities in medication access and adherence, designing the AI system to incorporate socioeconomic and health literacy considerations.

Recruitment Details
Participation Information:
The study is conducting a mixed methods study to develop an AI system for patients with diabetes at UAB Medical Center and Cooper Green Mercy Health Services.
Eligibility Criteria:
Participants must have a diabetes diagnosis and be at risk for medication discrepancies with recent (< 6 months) hospitalization or ED visit.
